Child and Family Services
Philosophy
At La
Frontera, a child and family team approach is used for all children
and families seeking services. This approach emphasizes the
importance of family and the community in a child’s health and
development. The team consists of the child, his/her caregivers
(e.g., parents, guardians), and anyone else the family wishes to
include, such as a close relative, friend, teacher, CPS worker,
probation officer, or other individuals who play an important role in
the child’s life. All team members work together to create a service
plan that reflects the family’s unique wishes, strengths, culture,
needs, and values. The team measures progress together until the
child and family’s goals are reached.
Services
-
Outpatient Counseling―Outpatient
behavioral health services are provided primarily in a clinic
setting with some services provided in the client's home, school, or
community. Service modalities include intake/assessment of the
behavioral health needs of the child and her/his family, individual
therapy and rehabilitation, family therapy, group therapies, family
support services, and facilitation of child and family teams.
Psychiatric services include evaluation and medication management.
Facilitation and case management services are provided for families
of children who need more restrictive levels of care.
-
Meet Me Where I Am―La Frontera Center provides in-home and community-based support and rehabilitation services to enrolled children/youth and their family members. These services are based on the identified needs in the child/youth's current service plan. Support services are focused on helping children/youth remain at home and in their own community.
Support staff provide these services to families in their own environment and in their preferred language. All services are coordinated by the Child and Family Team and are individualized to the needs, strengths, and culture of each family. Support services are provided with the intensity, frequency, and duration needed by each family and are based on positive actions and opportunities.
La Frontera recognizes the unique needs of families and can arrange for staff to be available to provide support services as needed to include later evening hours and weekends.
